- Your valid passport + 1 photocopy YOUR PASSPORT MUST HAVE AT LEAST TWO BLANK VISA PAGES REMAINING

- Your permanent resident card + 1 photocopy (if applicable)

- 2 recent passport size photographs

- 2 application forms filled out and signed

- Arrete de nomination + 1 photocopy,

- Your flight itinerary + 1 photocopy
