Your passport which must be valid for at least three months after your return from France YOUR PASSPORT MUST HAVE AT LEAST TWO BLANK VISA PAGES REMAINING + 1 photocopy

If applicable, your U. S. permanent resident card or valid US visa + 1 photocopy

1 long stay visa application form (if you’re staying more than 90 days in France) or 1 short stay application form (if you’re staying less than 90 days in France) filled out completely and signed: ONE APPLICATION FORM FOR EACH PERSON TRAVELING TO FRANCE

1 recent passport size photograph

An "CONVENTION D’ACCUEIL" or "PROTOCOLE D’ACCUEIL" issued by the host institution in France (must be written on their stationary paper), bear their seal, signature of the person in charge of this institution, and the seal of the French administration "PREFECTURE" + 1 photocopy

Your itinerary or plane tickets (round trip for a stay under 3 months or one way for stay over 3 months) + 1 photocopy

If your spouse and your children are accompanying you, in addition to the documents listed above (except the Convention d’Accueil or Protocole d’Accueil), they must provide a marriage certificate and a birth certificate + 1 photocopy
